Purpose: This indicator assesses the presence of processes that promote quality use of medicine for newly admitted residents to residential care homes (RCHs).

  • INDICATOR 3: Data collection tool – [Excel]

This indicator is based on recommendations for medication management made in the Guiding Principles for Medication Management in Residential Aged Care Facilities (Guiding Principles) to support aged care provider organisations, residential care homes (RCHs) and aged care staff to improve medicine safety. In addition to medication history-taking and reconciliation, it is recommended that RCHs provide person-centred care, respond to residents’ needs and preferences, guide residents who self-administer their medications and support residents who have swallowing difficulties. These processes are well placed to occur during the admission process.
